Fair Acres Geriatric Center is committed to enhancing the quality of life for our residents by strengthening overall independence and individuality. The Nursing Department works together with open communication and teamwork to provide a continuum of care to better meet the needs of the residents living in our community. We take a proactive approach to the assessment and evaluation of satisfactory outcomes to improve quality of life and quality of care for our residents.
The Nursing Department supports this mission by providing 24-hour skilled nursing care with highly qualified nursing staff, which consists of Registered Nurses, Licensed Practical Nurses and Certified Nursing Assistants.
All residents are evaluated, upon admission, and routinely by a full range of interdisciplinary team professionals to establish an individualized and comprehensive plan of care. This interdisciplinary team includes the Nursing Department, Social Services Department, Dietary Department, Activities Department and Rehabilitation Department. Residents and family members are always placed at the center of this team to best meet the needs of each individual resident and empower them to participate in their plan of care. The plan of care is continually reevaluated to reflect any potential changes.
Whether your loved one is independent or dependent with activities of daily living, we are here to support that need. The Nursing Department offers a wide range of routine services: dementia units with experienced dementia-care nurses, wound management by a Certified Wound Care Nurse, intravenous therapy, tracheostomy care with a Certified Respiratory Therapist on staff, a restorative nursing program, respite services and hospice and palliative care nursing services. Education is provided for the Nursing Department on an on-going basis to ensure nursing policies and procedures are current with evidence-based practices and industry standards of care.
